IMPORTANT DOCUMENTS TO PREPARE:

 

Equipment List - Please send us a list of your equipment including the year, make, model, VIN# and value.

 

Driver List - List your driver information including their name, date of birth, date of hire, years of experience and CDL#.

Commodities By % - List what you're hauling (ex. 95% Dry Goods, 5% General Materials).
 

IFTA - The last four quarters of IFTA reports.  This is important for long haulers.

 

MVRs - Any current MVRs processed within the last 6 months. 

 

Loss Runs - Last (4) four years of loss history. You can request from your current agent or contact your carrier directly. Must show a “run date” within 90 days of expiration.
